At A Plus Family HealthCare, we're proud to be an FQHC, or Federally Qualified Health Center. But what does that mean for you and your family? It means we're here to serve everyone, and we offer a Sliding Fee Scale Program to ensure affordability for all. FQHCs are community-based healthcare providers that receive federal funding to offer high-quality healthcare services to underserved populations. That includes comprehensive medical care, mental health services, and dental referrals, all in one place. We focus on providing care to everyone in our community, from children to seniors, with a strong commitment to preventive care and wellness. Our goal is to make sure you and your loved ones can access the care you need, when you need it, in a welcoming and compassionate environment. So, when you visit us, you're not just getting healthcare – you're getting care that's centered around you and your community, with affordability in mind.

Family Practice

  • General HealthCare for the entire family

  • Annual Wellness visits, Acute Visits for established patients

Pediatrics and Well Child Care

  • Providing care from birth to adolescence

  • Well Child Exams, Immunizations, School/Sports Physicals

Behavioral Health

  • We provide psychiatric services through evaluation, diagnosis, and medication management for individuals with mental health disorders.

  • Integrated Care Coordinated care that addresses both physical and behavioral health needs.

KidCare

  • Our goal is to improve the overall health of students to support their academic success.

  • Primary Care, Preventative Services, Acute Care, Referral and Care Coordination

  • We provide convenient telehealth services remotely via videoconferencing, allowing parents to participate in the virtual visit.

Immunizations, Labs and X-Ray Services

  • Immunizations for all populations.

  • We offer a broad range of laboratory services to meet the needs of the communities. *Some laboratory tests require prepayment. Sliding Fee Sale is available.

Translation and After-hours Care

Translation services are provided in many languages. After-hours call services are provided to ensure prompt attention to our patient’s needs
