Fee Collection
All copays, fees, and patient balances shall be collected at the time of service. Payment plans are available under extreme circumstances, but patient must first meet with a financial counselor/office manager.

There is an additional $25 dollar fee for completion of forms for use outside of the patient's medical record, including:
