Position Summary
Responsible for keeping the clinic organized and operating efficiently. Assists medical providers with charting and helps to meet quality guidelines.

Responsibilities
Conduct physical exams, diagnostic assessments, and routine health screenings
Diagnose and manage acute and chronic conditions
Develop and implement individualized treatment plans
Provide preventative care including immunizations and health education
Document patient encounters accurately in eClinicalWorks (EHR system)
Collaborate with physicians, behavioral health providers, and support staff
Participate in quality improvement initiatives and performance metrics
Ensure compliance with HRSA, FQHC, and clinical documentation standards
Required Qualifications
Active APRN license in the state of Kentucky (required)
National certification in an advanced practice specialty (e.g., FNP, AGNP, PNP, PMHNP, etc.)
Master's or doctoral degree in nursing (required)
Valid DEA license or eligibility to obtain
BLS certification (required)
Experience using electronic health records (eCW preferred)
Ability to work independently and as part of a care team
Strong clinical, organizational, and communication skills
Preferred Qualifications
Minimum of 2 years of experience as an APRN in a primary care setting
Prior experience working with underserved or rural populations
Interest in quality improvement and community health initiatives
